Determination of Factors Affecting the Readiness of Orthopedic Surgery Patients for Hospital Discharge

Özet

Bu çalışma, ortopedi cerrahisi geçiren hastaların taburculuğa hazır olma durumlarını ve etkileyen faktörleri belirlemek amacıyla yapıldı. Tanımlayıcı tipteki bu çalışma, İstanbul ilindeki bir vakıf üniversitesi hastanesinde Ekim 2019-Mart 2020 tarihleri arasında total kalça protezi (TKP) ve total diz protezi (TDP) cerrahisi geçiren 125 hasta ile gerçekleştirdi. Veriler ‘Hasta Bilgi Formu’ ve ‘Hastaneden Taburcu Olma Hazıroluş Ölçeği/Kısa Form (HTHÖ/KF)’ kullanılarak toplandı. Verilerin analizi SPSS 25.0 programında sayı, yüzde, ortalama, standart sapma, Mann Whitney-U testi, Kruskal Wallis testi ve Lojistik Regresyon Analizi kullanılarak yapıldı. Verilen taburculuk eğitiminin yeterli olduğunu (p=0.023) ve eve gitmeye hazır olduğunu (p<0.001), taburcu olunca tek başına rahatlıkla yürüyebileceğini (p=0.013) ve rahatlıkla yataktan/sandalyeden kalkabileceğini (p=0.004) düşünen hastaların HTHÖ/KF ölçeği puan ortalamalarının daha yüksek olduğu belirlendi. Daha önce ortopedik cerrahi geçirmeyen hastaların bilgi (p=0.004) ve baş etme (p=0.034) alt boyut sıra ortalamalarının ortopedik cerrahi deneyimi olmayan hastalara göre daha düşük olduğu bulundu. Çalışmanın sonuçları ortopedik cerrahi hastalarının taburculuğa hazırlık konusunda desteğe ihtiyaç duyduklarını göstermekte olup, hemşirelik bakımının hastaların taburculuğa hazırlığını destekleyici girişimler göz önünde bulundurularak planlanması oldukça önemlidir. 

This study was conducted to determine the discharge readiness of patients undergoing orthopedic surgery and the factors affecting it. This descriptive study was conducted with 125 patients who underwent total hip prosthesis (THP) and total knee prosthesis (TKP) surgery between October 2019 and March 2020 at a private university hospital in Istanbul. The study’s data were collected using the 'Patient Information Form' and 'Discharge from Hospital Readiness Scale/Short Form (RHDS/SF)'. The data were analyzed using numbers, percentages, means, standard deviations, Mann Whitney-U tests, Kruskal Wallis tests, and Logistic Regression Analysis in SPSS 25.0 program. It was determined that the mean scores of the HTHÖ/KF scale were higher in patients who thought that the discharge training given was sufficient (p=0.023) and that they were ready to go home (p<0.001), that they could easily walk alone when discharged (p=0.013) and that they could easily get out of bed/chair (p=0.004). It was revealed that the mean of information (p=0.004) and coping (p=0.034) sub-dimension scores of patients who had not previously undergone orthopedic surgery were lower than patients without orthopedic surgery experience. The results of the study show that orthopedic surgery patients need support in preparation for discharge and it is very important that nursing care is planned by considering the interventions that support the preparation of patients for discharge.
