The use of Non Invasive Ventilation in hospitalized patients with Palliative Care: a cross-sectional global survey.

This study evaluate the acceptability of NIV among physicians involved in palliative care. Methods: ethics approval, Clinical Trials Registry of India (CTRI/2023/06/054191), an online questionnaire, validated by the Delphi method, was distributed to members of the Association of NIV, Asia-Pacific Hospice Network, and Indian Association of Palliative Care. Results: A total of 210 responses were received. After excluding responses from non-medical specialists, inexperienced doctors, and centers without access to NIV, 157 responses from 41 countries were analyzed, India (47.3%), Italy (8.7%) and Malaysia (5.3%), majority (80.9%) were anesthesiologists, palliative care physicians, pulmonologists, or intensivists, with a nearly equal gender distribution (54.8% male, 45.2% female), 89.2% were involved in either part-time or full-time palliative care, and (62.4%) had received some training in managing terminally ill patients. All participants agreed that NIV is a useful tool for managing breathlessness in terminally ill patients, and most common indication being to decrease the work of breathing (59.2%). Regarding NIV utilization in the past year, 30.6% of respondents had not used NIV, 21.0% had used it in fewer than five patients, 19.7% in five to ten patients, and 28.7% in more than ten patients. NIV use in patients with DNAR (Do Not Attempt Resuscitation) or CMO (Comfort Measures Only) orders, 45.2% supported its use, 39.5% were uncertain, and 15.3% opposed it. No significant correlation was found between NIV usage and hospital type (p = 0.648). Conclusion: This global survey indicates widespread awareness of NIV in palliative care across various medical specialties. However, its actual utilization remains limited.
