Adaptation and validation of the Niigata PPPD questionnaire in Turkish: A clinical study

Özet
Amaç
Bu çalışma, Niigata Süreğen Postüral-Algısal Baş Dönmesi Anketi’nin (NPQ) Türkçeye çevrilmesi ve kültürlerarası uyarlanmasını yapmayı ve Süreğen Postüral-Algısal Baş Dönmesi (PPPD) olan Türkçe konuşan hastalarda güvenilirlik, geçerlilik ve tanısal doğruluk gibi psikometrik özelliklerini değerlendirmeyi amaçlamıştır.
Yöntemler
Bu prospektif vaka-kontrol çalışmasına 114 katılımcı dahil edilmiştir: 36 PPPD hastası, diğer vestibüler bozukluklara sahip 40 hasta ve 38 sağlıklı kontrol. Katılımcılar NPQ’nun Türkçe versiyonunu (T-NPQ) ve Baş Dönmesi Engellilik Envanteri’ni (DHI) doldurmuştur. İç tutarlılık Cronbach alfa katsayısı ile değerlendirilmiş, test-tekrar test güvenilirliği ise 14–30 günlük aralıkta incelenmiştir. Yapı geçerliliği, varsayılan üç faktörlü yapıyı (Dik Postür/Yürüme, Hareket ve Görsel Uyarım) test etmek amacıyla doğrulayıcı faktör analizi (CFA) ile değerlendirilmiştir. Yakınsak geçerlilik ise T-NPQ skorları ile DHI skorları arasındaki korelasyon incelenerek değerlendirilmiştir. Tanısal doğruluk, Alıcı İşletim Karakteristiği (ROC) eğrisi analizi kullanılarak değerlendirilmiştir.
Bulgular
T-NPQ mükemmel düzeyde iç tutarlılık göstermiştir (Cronbach α = 0.919) ve güçlü test-tekrar test güvenilirliğine sahiptir (r = 0.832, p < .001). Doğrulayıcı faktör analizi varsayılan üç faktörlü yapıyı desteklemiş ve iyi model uyumu göstermiştir (χ²/df = 1.47, CFI = 0.964, RMSEA = 0.064). T-NPQ toplam skorları ile DHI toplam skorları arasında güçlü bir korelasyon bulunmuştur (r = 0.709, p < .001). T-NPQ, PPPD hastalarını diğer vestibüler hastalardan ayırt etmede 0.746 Eğri Altında Kalan Alan (AUC) değeri göstermiştir. En uygun kesme noktası 27.5 olarak belirlenmiş olup duyarlılık %86.1 ve özgüllük %57.5 bulunmuştur.
Sonuç
NPQ’nun Türkçe versiyonu güvenilir ve geçerli bir araçtır ve PPPD hastalarında semptom şiddetinin değerlendirilmesinde faydalı olabilir. Türkiye’de klinik uygulamada ve araştırmalarda tarama ve sonuç ölçümü için etkili bir araç olma potansiyeline sahiptir.

Conclusion
The Turkish version of the NPQ is a reliable and valid tool that may be useful for assessing symptom severity in PPPD patients. It has the potential to be effective for screening and measuring outcomes in clinical practice and research in Turkey.
