A spermatocytic tumor is a rare tumour that has only been found in 1–12% of patients presenting with a testicular germ cell tumour. The tumor metastasizes only in few cases, having more chance when sarcomatoid or anaplastic transformation is seen. Orchiectomy without radio- and chemotherapy has been advised as standard therapy. In this case report, we describe a 68-year old patient with a testicular mass in his left hemiscrotum. An ultrasound showed two hypo-echogenic lobulated nodular laesions. Normal serum tumor marker level was observed. An inguinal orchiectomy was performed. Histopathological examination showed a spermatocytic tumour. Knowledge about a spermatocytic tumour and its hallmarks are essential. A short follow-up with CT and physical examination is advised.
